New Hire Employment Documents
Welcome to Robert Morris University!
The completed and signed documents below are required to be submitted to Human Resources (Revere 2nd Floor) on or before the FIRST DAY of employment.
Employment Eligibility Verification Form (I9): Pages 7 and 8 of this form must be completed and original identification documents personally provided on or before the first day of employment.

Direct Deposit Enrollment: Whether or not you elect to have direct deposit, this form needs to be completed and signed.
W-4 Form: Please complete items 1-7.
Residency Certification Form: Complete the top (Employee Information-Residence Location) and lower (Certification Signature) portion of the form. To determine the PSD Code for your local municipality, visit http://community.newpa.com. Please contact Payroll at 412-397-6280 with questions.
FACULTY Parking Permit Registration/STAFF Parking Permit Registration:To retrieve your parking pass, please return this completed form to Public Safety, Barry Center.

University and Human Resource Policies - Acknowledgement Form: Please review the University-wide Policies and Human Resources Policies; then sign and date the acknowledgement form.
Transcripts (Faculty Only): Full and Part Time faculty are required to submit an academic transcript of their highest degree to Human Resources. Nursing faculty are required to submit all degree transcripts. Please have official transcripts emailed directly to email@example.com or mailed to Robert Morris University, Department of Human Resources, 6001 University Boulevard, Moon Township, Pennsylvania, 15108.
Provisional Employment Disclosure Statement (FBI Clearance Required New Hires Only): If an FBI clearance has not been received by the first day of employment, please sign this form and return it with your new hire paperwork.
